What is the cost?
Our fees are based on the level of care provided and services you need. They are comparable to other clinics in the north state.
By eliminating the middleman (insurance companies) who profit off your healthcare dollars, we pass on the savings to you and provide you with quality care at affordable prices. This makes it easier for those with high deductibles or without insurance to also receive exceptional healthcare at an affordable price.
**If you do have insurance, we will be happy to provide you with a super bill that you can submit to your insurance; however, we do not make any guarantee of reimbursement.**
What is the "new patient" procedure?
As a new patient, you will complete a brief one page intake form that allows us to collect the necessary information to set up your patient profile in our electronic medical record. In addition we require a signed "authorization to treat" consent. We will personally take your medical history as we get to know you as a patient and will not ask you to complete lengthy medical history forms or use a computer to complete this information.
Even though we do not accept insurance, if you have medical insurance, please bring your card and we will scan it into our system. This information is helpful for necessary referrals such as physical therapy, imaging studies, labs or referrals to medical specialists.
